Re-Enrollment Forms

Please submit your forms on-line by simply clicking on the blue submit by e-mail tab after completing them.  There are three required forms with a variety of optional forms.  Please be reminded that all three of the required forms must be submitted.  You will receive a e-mail from the school office confirming receipt of your re-enrollment materials within 5 working days.

 

Family Registration & Emergency Information >
This form is REQUIRED and provides the necessary information to enroll your child for the upcoming school year.

Student Medical History>
New form REQUIRED for all families to complete. Please complete one per student.

Parent Consent Form>
This form is REQUIRED details consent for field trips, athletic participation, internet usage, and the like.

Emergency Preparedness>
This form is included with the Earthquake Kit. It should accompany the child's Earthquake Kit that is brought at the beginning of each school year. Please complete one per student.

Piano Registration>
*Optional -- for those families interested in the piano program. Please complete one per student.

Instrumental Program Registration
*Optional -- for those families interested in instrumental lessons. Please complete one per student.

Medication Request>
* Optional. Please complete one per student.
