Enrollment Forms for Prospective Parents

There is no cost to apply to Apostles. Fill out the application and teacher recommendation forms below. Please submit the forms to the school office. Forms may be mailed, faxed, or brought to the office.

Age requirements for enrollment:

• To enter our K1 program, children are generally five years old before the end of the school year.
• To enter our K2 program, children must be 5 years old on or before September 2nd.
• To enter first grade, children must be 6 years old on or before September 2nd.
